Your "student identifier" is a family identifier. Please use the following, regardless of the number of children you have enrolled. Oldest student last name, oldest student first name, middle initial. |

The person "Responsible for Payment" is the one who gives his/her financial information for tuition payment (usually the parent/guardian). |

Bank information: Only routing and account number information is required. Voided check, bank address and phone number are not necessary. |

The payments dates are the 5th or 20th of the month - parents' choice. Ten monthly payments begin in July, 2010 and go through April 2011. The months for semi annual or semester payments are July & December. |

"Payment Terms" will be filled in by the school office. |

The "Peace of Mind" insurance fee, if you choose this option, and the FACTS fee will be billed separately by FACTS. Do not attach a check! |
